Children’s long version questionnaire for age group I (4-7years containing 21 items), age group II (8-12 years containing 64 items) and age group III (13-16/18 years containing 77 items) Parent’s long version questionnaire, corresponding to the three age groups (age group I: 4-7, age group II: 8-12, age group III: 13-16/18 years) Children’s short version for age group I (containing 16 items) and for age groups II/III (identical for both age groups: containing 35 Items) Parent’s short version for age group I (containing 16 items) and for age groups II/III (identical for both age groups: containing 35 Items) Children’s Index version identical for all age groups (containing 8 items) Parent’s Index version identical for all age groups (containing 8 items)
